Dear Clients,

 

We wanted to reach out and give reassurance in our community. Our office at Vital Massage has always been above standards when it comes to sanitation protocols; to keep our clients/staff healthy and safe. 

 

Sanitation of the entire office including doors, desk, chairs, ect. Sanitation of treatment rooms before/after clients to include, benches/seats, table, face-cradle, handles/nobs, doors, oil container, massage tools, ect. Our linens are also a step above when keeping our staff and clients healthy. Linens are washed to be hypoallergenic, as well on an extra high heated sanitation setting, then set to dry using a high heat to thoroughly dry and sanitize.

With our sanitation standards being set above standard, and our staff being even more diligent and adding frequencies of sanitation rotations throughout the office, we are even more focused in keeping clients/staff healthy and safe. â€‹

Payment Agreement: Before we begin services, you will indicate you have read, understand and agree to the following payment policies. *You agree to be financially responsible for all charges regardless of any applicable insurance or benefit payments, third-party interest, or the resolution of any legal action or lawsuits in which you may be involved. *Payment is expected at time of service unless you have made other payment arrangements with us. *Using Insurance. You are responsible for contacting your insurance company to determine what your benefits are and obtain any necessary physician referrals and/or pre-authorizations for services. We are not responsible if your health plan denies, in whole or in part, your claims for our services. You are responsible for the balance owed for all services received.

Cancelation Policy: *Late Cancelation. 48hour or more notice is required to cancel an existing appointment. A minimum $60 per hour scheduled will be charged for late cancelations(Less than 48hour notice. *No Show. If you do not arrive for your appointment a minimum $80 per hour scheduled will be charged for no show. *Service Packages. If you purchase a package, and you have a late cancelation or no show to an appointment you have scheduled, that appointment will be counted as one used from your total for your purchased package. This ONLY applies to packages purchased; if you did not purchase a package you will owe the corresponding late cancelation or no show fee.
